Seeing a need in preventing disease rather than just treating it once it occurs, Dr Krikorian transitioned to preventive cardiology from interventional cardiovascular procedures. This focus involves a comprehensive, patient-centered approach emphasizes reducing the risk of atherosclerotic cardiac and vascular disease through a combination of lifestyle modifications and, when indicated, pharmacologic therapy and nutraceuticals.

The American College of Cardiology and American Heart Association recommend prioritizing lifelong promotion of a healthy lifestyle, including a diet rich in vegetables, fruits, whole grains, primarily plant based and some lean animal based proteins (fish, poultry), and minimizing intake of processed trans fats, red meats, refined carbohydrates, dairy and sugar-sweetened beverages. Regular physical exercise, including a combination of resistance training and aerobic activities, for at least 150 minutes per week at moderate-intensity or 75 minutes per week of vigorous-intensity exercise, is strongly advised. Tobacco avoidance and cessation along with avoiding all kinds of substance abuse are also critical components. Our practice promotes good sleep, stress control, and dental health in order to acquire the overall positive approach to life and stay healthy and content for a longer time. Risk assessment in Dr Krikorian’s practice is central: adults aged 40–75 should undergo 10-year Atherosclerotic Cardiovascular Disease [ASCVD] risk estimation. Our shared decision-making, will guide initiation of pharmacologic interventions such as diabetes control and management, antihypertensive therapy, statins and other lipid lowering meds and supplements or aspirin, perhaps meds and supplements decreasing inflammation and the absolute risk and risk-enhancing factors. For patients with type 2 diabetes, lifestyle changes are first-line, with metformin as initial pharmacotherapy if needed. We emphasize team-based care, assessment of social determinants of health, and ongoing monitoring of progress and barriers to change. The rationale for these recommendations is that most ASCVD events are preventable through early intervention targeting modifiable risk factors, and matching the intensity of preventive efforts to individual risk optimizes benefit while minimizing harm.

Dr Krikorian’s office focuses on precision medicine by measuring initial risk utilizing CT scan based Coronary Calcium Score, Carotid Intima Media Thickness (CIMT), which is an ultrasound test and other modalities for better disease assessment like Coronary CT Angiogram, stress tests and others. Based on those test results, extensive blood work might be needed to assess all pathways for plaque build-up by going beyond the traditional and more limited lipid testing.
